Local NHS providers are planning how they can begin delivering more services again and are asking residents to fill in a survey with their experiences of COVID-19.

This could be because you have had the virus or about the services you would normally use. Tell us what’s worked well, what could be better and how you would like to see services delivered in the future. If you have had coronavirus, you can tell us about any support you received too.

The information you provide will be stored safely and will be used anonymously to help us work to improve local services. It will take about 5 minutes to complete.
